Patient Care Coordinator Albany, OR Specialty: Patient Coordinator Position Type: Travel Contract Length: 13 weeks Pay: $1871 - $2254 | Shift: 5x8 Days Are you a compassionate professional with a background in social services, counseling, or psychology? Do you have a passion for supporting patients and their families through the final stages of their journey? If so, we have an exciting opportunity for you to join a dedicated hospice team providing essential care in a home setting. As a Patient Care Coordinator, you will play a vital role in ensuring that patients receive the highest quality of care and support. You will work closely with a multidisciplinary team to deliver comprehensive services, including skilled nursing visits, pain and symptom management, and emotional and spiritual support.
Coordinate patient care plans and ensure seamless communication among healthcare providers Provide grief and loss counseling to patients and families Utilize your personal vehicle for home visits, maintaining a valid driver's license and insurance Need 1 year experience in setting Bachelor's degree in social services, counseling, or psychology required; Master's degree preferred Healthcare Provider Level BLS required Experience or education in grief or loss counseling required Apply today and let Capstone Health help you reach your full potential!
