Organizes the clinical program
Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population
Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.
Serves as resource to hospital staff regarding patient care.
Coordinates the patient evaluation process.
Performs initial assessment of patient and family according to protocol for acceptance into program

Graduate of accredited school of nursing
Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical / surgical setting
Registered Nurse: AZ
Preferred
Bachelors of Nursing or higher degree
Critical care experience
Program Management/Development experience
Care Coordination experience
