Required Qualifications
Physical Therapist, Skilled Experience: 1 year Licenses: PT(Compact Accepted) Certifications: BLS(Copy Needed) SSN Required DOB Required References: 1 Reference in entire work history License and SNF experience are required. Preferred Qualifications
