Direct Support Professional Needed

Caregiver for individuals in the agency.

Characteristic duties include:

• Assist the individual in his/her as documented in the ISP, Service Plan etc.
• Routine hygiene care throughout the day.
• Medication reminder
• Supervise in addressing the ISP/Plan of Care goals and document progress
• Support all opportunities for the growth, learning and development of the individual
• Complete daily progress notes highlighting person center events
• Complete understanding of home policies and procedures including home rules, disaster procedures, participant's rights, etc.
• Document progress notes before leaving the job site
• Document tracking of ISP goals prior to leaving the job site
• Ensure the individual is integrated into the community
• Notify the agency of any concerns regarding the individual
• Attend all staff meeting (required), participate in scheduled training sessions including on the job training
• Promote harmonious relationship with other staff members
• Maintain individual's records on site in accordance with policy and procedures
• Other duties as assigned

MUST HAVE THE FOLLOWING:

• CPI
• CLEAN 7 YEAR MVR
• NATIONAL BACKGROUND CHECK (ONE TIME $60 FEE)
• RESUME (MUST REFLECT EXPERIENCE WITH DBHDD CLIENTS)
• CURRENT PHYSICAL
• CURRENT TB TEST
• CERTIFICATES OF TRAINING
• CURRENT DRIVERS LICENSE
• AUTO INSURANCE
• COPY OF PROFESSIONAL LICENSE
• COPY OF SOCIAL SECURITY CARD
• FIRST AID CERTIFIED(required)
• CPR CERTIFIED (required)
