Rosewood, a distinguished award-winning HumanGood life plan community in Bakersfield, CA is seeking a Full-time LVN Case Manager. for Health Center (skilled nursing facility). The LVN case manager works with new residents to assess their conditions and requirements with the purpose of developing care plans customized to meet the specific personal needs.
** Full-time, Monday - Friday, 9:00 AM - 5:00 PM ** $30.00 - $34.00/hour (DOE) Key duties and responsibilities include, but may not be limited to:
Working with new residents to assess their conditions and requirements along with the purpose of developing care plans customized to meet the specific personal needs of residents; Assuming primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals and interventions; Coordinating the implementation of the plan of care and insure care coordination and case management for each patient. As applicable, apply specific criteria for admission and re-certification to establish appropriate levels of care and patient's eligibility; Assesses resident's needs at the time of move in and continually evaluates residents' condition. Promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community's safety procedures) Developing discharge plans and ensures quality cost effectiveness. Developing assignments in coordination with the care partners and assist in supervising team members that are accountable for resident care and satisfaction. What you need to be successful in this role
Current California state LVN license Current CPR certification 2 years case management experience Minimum of 2 to 4 years professional managerial/supervisory experience in a related field Excellent clinical observation and assessment skills Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members
