You'll make an impact by: Partner with nephrologists, dialysis teams, and interdisciplinary care providers to deliver comprehensive dialysis care, address patient concerns, and ensure accurate, timely clinical documentation.
Manage chronic kidney disease patients across outpatient settings by conducting assessments, developing treatment plans, and coordinating ongoing care in collaboration with physicians.
Facilitate seamless transitions of care through annual wellness visits, post-hospital follow-up, medication reconciliation, and patient education to improve outcomes and reduce readmissions.
Provide advanced clinical care across multiple settings-including physician offices, skilled nursing facilities, rehabilitation centers, dialysis units, and on-call environments-through diagnostic evaluations, treatment planning, and evidence-based interventions.
Prescribe medications and therapeutic treatments within scope of practice while delivering compassionate, patient-centered care and maintaining the highest standards of professionalism and clinical excellence. What You'll Bring Master's degree in nursing or related field, or equivalent combination of education and experience (4-10 years) Current Arizona RN license with Nurse Practitioner (NP) or Physician Assistant license Proficiency in Microsoft Office and experience with ICD-9 and CPT coding Strong organizational skills, flexible schedule availability and reliable transportation for travel between sites A patient-first mindset and a commitment to providing compassionate care
