As an LPN, you will play an important role in supporting patient care, clinic operations, care coordination, and positive health outcomes for the diverse communities we serve. About the Role The Licensed Practical Nurse provides direct patient care and clinical support within an outpatient primary care setting. Working under the supervision of a Registered Nurse or licensed provider, the LPN supports safe, efficient, and patient-centered care while collaborating with providers, nurses, medical assistants, and care coordination staff. This role is ideal for someone who is clinically skilled, dependable, team-oriented, and committed to serving patients with dignity, respect, and compassion. What You'll Do Key responsibilities include:
Prepare patients for examination by obtaining vital signs, updating medical history, and completing medication reconciliation. Assist providers during examinations and procedures. Administer medications, injections, and immunizations according to provider orders and established protocols. Perform point-of-care testing and collect laboratory specimens as appropriate. Maintain accurate, timely, and complete documentation in the electronic health record. Support referrals, lab tracking, follow-up care, and care coordination activities. Assist with chronic disease management, including diabetes, hypertension, and asthma monitoring. Participate in outreach and follow-up efforts to support patient adherence to care plans. Support preventive care initiatives, including screenings and immunizations. Collaborate with providers, registered nurses, medical assistants, and care coordination staff. Communicate patient concerns, changes in condition, or barriers to care with the appropriate team members. Follow clinical policies, infection control guidelines, HIPAA requirements, and safety standards.

Work Environment This role operates in a clinical environment with frequent interaction with patients, families, and interdisciplinary care teams. The position requires the ability to stand, walk, move throughout the clinic, assist patients as needed, and manage multiple tasks while maintaining a high level of patient care and service. Travel between clinic sites may be required based on organizational needs. Required Qualifications
Graduate of an accredited Practical Nursing program Current Licensed Practical Nurse license in the State of Ohio Basic Life Support certification Strong communication and patient care skills Ability to work effectively in a team-based clinical environment
Preferred Qualifications
One year of clinical experience Experience in primary care, community health, or ambulatory care Experience using electronic health records
