Requirements: - 10% Salary placement fee - Salary range: $42-45 - 35 hours - M0F - Weekend rotation
Roles & Responsibilities: - Provide patient care in long-term care and rehabilitation settings. - Assist with daily living activities and support the rehabilitation process. - Collaborate with multidisciplinary teams to develop and implement patient care plans. - Monitor patient progress and report changes to the healthcare team. - Educate patients and families about health conditions and treatment plans. - Maintain accurate and timely documentation of patient care activities.
