Description: Plans, develops, and provides diet interventions, medical nutrition therapy, nutrition care, and preventive counseling to individuals and groups. Delivers services across all areas of patient/resident care, including acute care, skilled nursing, outpatient, and community settings. Monitors nutritional care of clients/patients/residents of all ages, including neonatal, pediatric, adolescent, adult, and geriatric populations as assigned. Determines nutritional status through nutritional assessment and develops individualized care plans. Makes provider order recommendations and implements nutrition therapy, including verbal orders, diet modifications, and nutrition support. Provides education and counseling for disease and nutrition risk management while evaluating the outcomes of medical nutrition therapy interventions. Optimizes self-management education and skills for clients/patients/residents with chronic conditions. Recognizes, evaluates, and addresses nutritional needs based on cultural, ethnic, and religious beliefs. Evaluates treatments or procedures for effectiveness using an evidence-based approach. Works on nutrition care protocols for the assigned area and leads the healthcare team in implementing nutrition therapies. Educates on diet according to disease state, medical condition, and health risk. Open to flexible scheduling options. Cancellation policy allows for client cancellation up to 12 hours every 13 weeks.
