Contract Length: 14 weeks Guaranteed Hours: 40 per week
Requirements:
Current State License as a Physical Therapist Proof of current CPR certification Current driver's license. Must have reliable transportation and insurance 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting Previous home care experience - Preferred Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities:
Perform clinical assessments to identify patient needs and suitability for therapy. Document, prepare/review, and complete Plans of Care (POC), progress notes, and other clinical records. Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on evaluations and medical referrals. Instruct patients and families on safe transfer techniques and equipment use. Educate patients and families on therapeutic procedures tailored to their specific needs. Ensure adherence to infection control protocols during patient care activities.
