Overview Shift: 8:00 AM - 4:30 PM | 3 days/week (+ every 4th weekend) Provide compassionate care and make a meaningful difference in patients’ lives as a Home Health/ Hospice Aide. In this role, you’ll deliver high-quality personal care, homemaker services, and companionship to patients in their homes while supporting the RN Case Manager to ensure coordinated, patient-centered care. Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility
Provide light housekeeping, meal preparation, and companionship
Support medication administration and simple procedures under RN guidance
Observe and report changes in patient condition or home environment to the RN Case Manager
Maintain accurate documentation and ensure compliance with care plans and regulatory standards
Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives
Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries
Qualifications Education:
Completion of a 120-hour Nursing Assistant course .
High School Diploma/GED High School diploma or GED
License(s)/Certification(s):
Valid Driver’s License (IA, IL, or WI) – Required upon hire
Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)
Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred
Child Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
Experience:
Completing of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CNA work after completion
