Case Management Registered Nurse Location: Eureka, California Assignment Duration: This is a 13-week contract position. Schedule: Day shift, 5x8-hour schedule (08:00 - 16:30), required to work every other weekend. Job Description: We are seeking an experienced Case Management Registered Nurse to join our team. Ideal candidates will have a strong background in acute care and possess the necessary skills to provide high-quality patient management and coordination. The role requires floating between like units as needed based on departmental requirements. Responsibilities:
Conduct assessments and develop comprehensive care plans for patients. Coordinate services across multiple disciplines to ensure continuity of care. Manage patient admissions, discharges, and transitions effectively. Ensure compliance with regulatory requirements and standards of care. Educate patients and families about care processes and available resources.
Required Experience / Certifications / Licensure:
Minimum of 2 years of experience in acute care nursing. Current RN license. Experience in case management, including familiarity with admission criteria, discharge planning, and care coordination. Knowledge of CMS regulations and familiarity with Epic charting system is required.
