Performing thorough patient evaluations, initiating diagnostic procedures, and developing tailored care plans Diagnosing and managing acute, episodic, or long-term conditions in tandem with physicians or independently (as credentialed) Providing education to patients and families about prevention, health, and disease management Prescribing treatments and medications within the scope of your practice Engaging in chronic care management and follow-up care for patients Collaborating with interdisciplinary teams to guarantee comprehensive and cohesive care Accurately documenting patient interactions in electronic medical records (EMR) Qualifications We Seek
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) Completion of an accredited Nurse Practitioner program Mandatory Licensure and Certifications
Active Nurse Practitioner license in the state of Minnesota National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or an equivalent specialty Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to earn within 90 days) Essential Skills for Success
A strong clinical judgment coupled with a compassionate approach to patient care Exceptional interpersonal skills with patients, families, and healthcare team members Flexibility in fast-paced, community-focused settings Proficiency with EMR technologies and comfort navigating telehealth systems A collaborative spirit and adherence to evidence-based practices What Makes This Role Shine
