Physical Therapist — Outpatient — Monument, CO Join a community-focused outpatient clinic in Monument, CO as a Physical Therapist. Evaluate patients, develop and implement individualized treatment plans, document care in compliance with regulations, and participate in student education and professional development. Uphold professional standards, promote patient and employee safety, and contribute to performance improvement initiatives. This role requires effective clinical reasoning, clear documentation, and collaboration with interdisciplinary teams. Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Colorado Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Experience in outpatient physical therapy and proficiency with 60-minute initial evaluations and treatment sessions. Ability to perform documentation, goal writing, letters of medical necessity, and charge entry in accordance with clinic policies and payer guidelines. Demonstrated competence in safe patient handling and materials handling procedures.
Preferred Qualifications
Experience supervising or mentoring students in a clinical setting. Familiarity with performance improvement processes and clinical outcomes measurement.
Assignment Details
Shift pattern: 5x8 schedule. Duration: 13 week assignment. May be required to follow clinic guidelines for appearance, attendance, and punctuality.
