Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent care based on nursing theory and research. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family, and team members. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Demonstrates allocation of material resources effectively.
Role models the tenets of Relationship Centered Care through various actions including admission welcome and bedside shift report. Considers the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance patient care perceptions. Initiates service recovery for patients and families who have concerns and escalates as needed. Demonstrates caring, respect, compassion, and empathy in all interactions. Conducts purposeful hourly rounding on assigned patients addressing key patient care needs.
Practices safe and efficient patient-centered care, identifying opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional evidence-based procedures. Engages in peer and institutional review processes for quality and safety practices. Contributes to quality improvement projects and nursing research studies. Documents nursing practice accurately and in compliance with standards of care. Facilitates patient throughput on shift basis.
Contributes to the environment of care to support clinical colleagues and patients. Advances clinical competence in nursing practice to progress from novice to expert. Projects a professional image and communicates with respect and compassion. Incorporates ethical principles into decision-making for patients and families.
