Overview Unit Notes:Type of Facility SNF/LTC Floor Mix of long term and skilled residents# of Beds/Residents 30 residents per unit (60 total for floor)private roomsEMR Point Click CareRatios RN's/LPN's: 1:15CNA's: 1:10Required Certifications CPR Must have their CPR prior to first dayCommon diagnosis / Types of patients Inability to care for self, Sepsis, Femur Fx, NSTEMI, CKF, CHF, UTIMix of long term residents and residents here to gain strength to return home Job Duties LPN - Pass meds, wound care, foley/cath care, PEG tube feedings, ostomy care, trache careRN will take care of IVs, PICC lines, mediports, etcExperience or 'Must have' skillsAre there IV's or titratable drips? IVs, PICC lines, Mediports, Central lines IV Antibiotics/Blood/ IVFArea there Tele monitors? If so do you have monitor techs on this unit? Do RN's read strips?NO Are RN's required to attend team conferences for acute inpatient rehab?N/ADON/Charge Nurse/House Supervisor Darla Kuri is DON; Always a charge nurse in facility on 2W Manager is Wende Lightner; Clinical Coordinator is Ashley ConwayPhysician availability after hours Physicians available to be called 24/7Interdisciplinary Team: PT/OT/SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians? PT, OT, ST, Social Services, Case Managers, Business Office, Dieticians, Pharmacist, Medical Records all on siteIs RT on-site 24/7Oxygen Therapy /Equipment (ie Cpap, Bipap, High Flow) NO Pharmacy on-site? Medication dispensing system?Pharmacist on site; medications received from hospital down the hillShifts & Scheduling (Do you allow self-scheduling?
