Job Summary Join our Aegis Team as a LPN Case where you will play a crucial role in delivering exceptional care to our patients. Your compassionate approach will ensure comprehensive physical, social, and psychological assessments, and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary TEAM in developing comprehensive care strategies and plans. With your attention to detail, you will efficiently coordinate patient care delivery, manage resources, and maintain meticulous patient records. What You’ll Do As a Full-Time LPN in Omaha (M–F, 8–5), you’ll support patients in their homes by delivering skilled nursing care with a calm, can-do approach. This role exists to keep care consistent, safe, and well-coordinated—so patients can focus on healing where they’re most comfortable. Key responsibilities include: - Administer prescribed treatments and medications - Support ADLs and pain management - Teach patients/caregivers practical self-care techniques - Observe patient status and report changes to the RN/physician - Join interdisciplinary care planning discussions - Keep documentation accurate and ensure needed supplies are on hand Required qualifications/skills: - Current Nebraska LPN license - Home Health/Hospice experience (preferred) - Strong communication, organization, and time management - Ability to lift up to 35 lbs and perform full-range motions - Valid driver’s license, reliable transportation, and proof of auto insurance If you’re ready to bring great care (and good energy) to patients at home, apply today. Your Typical Day You’ll start with a quick look at your schedule, then head out across Omaha to see patients where they’re most at ease—home. Between visits, you’ll stay in touch with the RN and the rest of the care team so everyone’s on the same page. Expect a steady rhythm of patient check-ins, caregiver conversations, and a little bit of “plan B” thinking when needs change (because they do). You’ll wrap up by getting your notes finalized and making sure tomorrow’s route is set—so you can clock out feeling organized and confident.
