Primary Care Physician

Oak Street Health seeks a Primary Care Physician to deliver comprehensive, preventative care to Medicare adults in underserved communities. In this team-based model, you’ll manage a dedicated patient panel, perform chronic disease management, wellness visits, and acute care, and coordinate with nurses, care managers, and specialists. You’ll use data-driven tools and value-based care principles to improve outcomes, reduce hospitalizations, and enhance patient experience in a supportive, patient-centered culture.
