Assess residents with existing or newly developed wounds. Collaborate with physicians to establish and implement individualized treatment plans. Lead weekly wound rounds with the care team; monitor and document wound progress. Maintain accurate weekly wound tracking forms. Communicate promptly with physicians when wounds are not responding to treatment. Prepare and distribute weekly decubitus statistics to the wound care team. Conduct in-service training for staff on wound care procedures. Perform quarterly Quality Assurance reports to ensure compliance with wound care standards. Observe and evaluate dressing changes to ensure staff competency. Provide education on wound care to residents and families as needed. Participate in continuing education to stay current with best practices.
Requirements:
Active New York State Registered Nurse (RN) license Wound Care Certification and/or Infection Control Certification preferred Minimum of 1 year of nursing experience (nursing home experience preferred)
