Schedule 5x8.0 schedule, requiring 40 hours per week including two evening shifts. Job Description The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans according to the patient's needs, goals, and physician directions. The role requires ensuring the design of functional treatment programs aimed at facilitating effective patient return to community activities. ResponsibilitiesConduct comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop appropriate treatment plans and goals. Document medical records in a clear, concise, accurate, and timely manner. Ensure care is provided under valid physician orders and within insurance approval. Participate in student programs, providing supervision and evaluations. Create individualized professional development plans with clinical management. Adhere to discipline-specific state practice acts and maintain active licensure. Engage in performance improvement initiatives to enhance service quality. Required Experience / Certifications / LicensureDegree in Physical Therapy from an accredited school. Minimum of 2 years of outpatient clinical experience. Current CPR certification.
