Deliver high-quality, patient-centered care in an FQHC Look-Alike setting through in-person visits and telehealth Conduct well-child visits, immunizations, developmental screenings, and acute care visits Diagnose and manage pediatric illnesses, chronic conditions, and behavioral health concerns Participate in a shared 24-hour call rotation with other providers Review lab results, referrals, and emergency room discharges, ensuring appropriate follow-up Collaborate with interdisciplinary teams including nursing, behavioral health, dental, and specialty providers Support education of patients, residents, medical students, and staff as appropriate Participate in quality improvement initiatives and maintain a continuous improvement mindset Maintain accurate, timely, and compliant electronic health records (EHR) Engage in community outreach activities as needed Ensure compliance with FQHC regulatory requirements and applicable funding/grant guidelines
Qualifications
MD or DO from an accredited institution Active, unrestricted Illinois medical license Board Certified or Board Eligible in Pediatrics Current NPI and DEA registration (or ability to obtain prior to start) Current BLS/CPR certification (PALS preferred) Experience or strong interest in community health and underserved populations Proficiency with Electronic Health Records (eCW experience preferred) Ability to work effectively within a multidisciplinary team (medical, dental, behavioral health, etc.) Strong communication and interpersonal skills Spanish language proficiency preferred
Compensation & Benefits
Salary range: $160,000 – $215,000 based on experience Comprehensive benefits package including:
