State License License must be in hand at time of application. Certifications ASHA (CCC-SLP) Certifications BLS or BLS (AHA) Specialty Experience 1+ years of experience in General in last 3 years. Specialty Experience 1+ years of experience in job's specialty. EMR Must have experience with EMRs. Skills Assessment GSP skills assessment with 2.7+ score. References 2 Supervisor references within the last 2 years. Additional Confirmations I have submitted all time off requests. I am aware that I cannot make additional requests after submission. Additional Confirmations Experience screening, evaluating, and rehabilitating patients with communication, cognitive, or swallowing disorders is required. Additional Confirmations Utilization of standardized assessments and evidence-based practice is required. Additional Confirmations Ability to supervise and support CFY or Student Speech-Language Pathologists is required. Additional Confirmations LTC or Rehab experience is required. COVID Vaccine Medical and religious declinations are accepted. Willing to obtain allowed. Flu Vaccine (Current Season) Must be for the current flu season. Medical and religious declinations are accepted. Willing to obtain allowed. Travel Contract Must travel at least 50.0 miles to receive a travel stipend. Local candidates are eligible, but the pay package may adjust the rate by a few dollars per hour, will not contain a travel stipend, and will be entirely taxable.
