Provide direct patient care, including assessment, medication administration, wound care, pain management, and symptom control. Develop and implement individualized care plans based on patient needs and preferences, in collaboration with the interdisciplinary team. Educate patients and families on disease processes, symptom management, and end-of-life care options. Document patient assessments, interventions, and responses accurately and thoroughly. Collaborate with physicians, social workers, chaplains, and other members of the hospice team to ensure comprehensive care. Oversees licensed vocational nurses and aides in the delivery of patient care. Maintain a safe and comfortable environment for patients and families. Adhere to all hospice regulations and standards of practice. Participate in on-call rotation as required. Attend interdisciplinary team meetings and contribute to care planning discussions. Maintain up-to-date knowledge of hospice care best practices and regulations.
Schedule Requirements:
Must sign-up for a minimum of one (1) 16-hour after-hours back-up On-Call shift per month/schedule period. After-hours back-up On-Call shifts are from 1700-0900. See On-Call Protocol. All Full-Time or Half-Time Hospice House Nurses must be willing to work two (2) weekend shifts (Fri. and/or Sat. NOC; Sat. and/or Sunday DAYS) each month, as needed. The per diem Hospice House Nurse must be available to work a minimum of two (2) shifts per month/schedule period. After-hours back-up On-Call shifts may be substituted
Qualifications Required Education/Credentials:
Graduate of an accredited nursing program. Current California Registered Nurse license.
Required Experience:
Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.
Required Skills and Abilities:
Strong clinical assessment skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate and patient-centered approach to care. Proficiency in electronic medical records (EMR) systems.
Preferred Qualifications:
Experience in hospice or home health. Experience working effectively with an interdisciplinary team. CHPN certification.
