In conjunction with the Program Director, supervise clinical staff and oversee the delivery of services to clients. Be available on-call for 24-hour crisis intervention. Attend meetings as requested and be willing to work nights and/or weekends, as needed. Assist in managing personnel activities, including hiring, shift scheduling, policy and procedure development, staff meetings, case conferences, staff training, performance evaluations, disciplinary actions, and labor/management issues. Provide case management, including benefits assistance, helping clients complete housing applications, linking clients to primary and behavioral health care in the community, and offering housing referrals to formerly homeless adults diagnosed with serious mental illness and/or substance use disorders. These services are primarily provided in the client's apartment and in community settings. Serve as a critical member of the treatment team, collaborating with other providers to diagnose struggles, develop and implement interventions, and evaluate clients to support their independence. Assist with medication observation and education in accordance with OMH regulations. Assist in identifying clients' strengths, needs, abilities, and goals, and create related Service Plans in the electronic client records system. Assist the Program Director with administrative duties, including data entry, monitoring performance measures, program audit preparation, and staff attendance. Motivate clients to develop skills necessary to maintain housing and comply with legal obligations. Assist and encourage clients to develop natural supports and incorporate them into their plans. Complete required documentation for every client contact in a timely manner, in accordance with agency policy. Ensure documentation meets regulatory guidelines for the level of care. Prepare documentation, including psychosocial assessments (and psychiatric evaluations, if licensed), for 2010E housing applications. Co-facilitate groups if requested in areas such as wellness, substance use, harm reduction, symptom management, and medication education. Deliver direct care services and act as an internal consultant in onsite and offsite emergencies, providing counseling, crisis intervention, and psychosocial assessments. Ensure compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation, and incident reporting. Liaise with other departments and outside agencies, including the agency's Critical Care team. Work with residents, families, and staff in a caring and respectful manner, considering cultural differences. Immediately report incidents, allegations, or sensitive situations to supervisors. Complete incident reports in accordance with Bridge policy. Account for residents, perform diligent searches, and file missing person reports on residents who are not accounted for, following policy and procedure. Attend staff meetings, supervision, and training sessions as required. Perform any other duties as may be assigned.
Qualifications
MSW or MHC with relevant experience, including supervisory roles; licensure preferred. Supervision hours are provided for clinical licensure. Experience working with mental health, crisis intervention, substance use, criminal justice, and managing trauma-triggered behavior. Excellent time management skills; stress-resistant, flexible, team player, open to feedback, creative; exceptional organizational skills. Willingness to perform CPR/First Aid/AED/Naloxone. Experience working with justice-involved individuals. Preferred knowledge of AWARDS. Hours: Full-Time, 35 hours per week, Tuesday-Saturday 9 am-5 pm, on-call. Available on weekends and evenings for coverage and emergencies, as needed. Bilingual Spanish is highly desired.
