Current Licensed Practical Nurse (LPN) license
Roles & Responsibilities:
Assist in identifying patient needs and priorities for care based on various factors including age and lifestyle. Gather, observe, record, and communicate patient responses to nursing interventions. Collaborate with licensed practitioners to modify plans of care as needed. Implement nursing interventions and document care accurately and timely. Communicate relevant patient information with the multidisciplinary team to ensure continuity of care. Participate in patient education and discharge planning.
