Job Description Title:
Schedule: Shift: 08:00 AM - 04:30 PM Days: Monday - Friday
Assignment Details: Contract Length: 52 weeks Guaranteed Hours: 0
Requirements: Salary range: $40 - $50 10% Salary placement fee
Roles & Responsibilities: - Provide patient care in accordance with established guidelines and standards. - Collaborate with rehabilitation team members to develop and implement care plans. - Monitor and document patients' progress and communicate changes to the healthcare team. - Ensure compliance with facility policies and procedures. - Participate in staff meetings and training sessions as required.
Compliance / Notes: - No weekend rotation. - Medium-sized skilled nursing facility with established rehab staff.
