Schedule Day Shift: 5x8 hours (08:00 - 16:30) Job Description A Home Health Physical Therapist is needed to provide individualized care to patients in their homes within the Medford, Oregon area. The position involves delivering high-quality physical therapy services that align with established standards to promote patient improvement and wellness. ResponsibilitiesConduct evaluations and develop treatment plans for patients. Deliver therapy interventions and monitor patient progress. Utilize OASIS for documentation and care planning. Educate patients and families on exercises and care techniques. Manage a caseload of up to 6 patients per day, adhering to productivity standards. Required Experience / Certifications / Licensure1 year of skilled home health experience required. Active physical therapist license in the state of Oregon.
