Provides care in accordance with the Nurse Practice Act and professional nursing standards. Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history. Develops, implements, and revises the Plan of Care based on ongoing patient evaluations. Administers medications and treatments as prescribed in the physician's plan of care. Educates patients and families on self-care, medications, and diet as appropriate. Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition. Coordinates care with physicians, interdisciplinary teams, and community health partners. Ensures a safe environment for patients and supports their activities of daily living. Provides emotional support and counseling to patients and families. Arrange for necessary medical equipment and supplies. Ensures HIPAA compliance and patient confidentiality.
Additional Duties
Participates in on-call visits as required including evenings and weekends. Attend interdisciplinary group meetings. Supervises ancillary personnel and delegates responsibilities as needed. Engage in professional development through training and continuing education. Contributes to quality assessment and performance improvement activities. Performs other duties as assigned.
Qualifications Requirements/Qualifications
Must have a current Registered Nurse (RN) license in state(s) of practice. Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact. Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice. Ability to pass DHS background, consumer report, and drug test. Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Pushing/Pulling and Lifting/carrying up to 30 pounds. Occasionally up to 50 pounds. Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
The starting salary range for this role is $73,000-$95,000 annual base salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
