Assess patients in inpatient or outpatient settings and craft personalized treatment plans tailored to their needs Implement physical therapy modalities aimed at enhancing mobility, strength, balance, and overall function Engage collaboratively with nursing staff, physicians, case managers, and rehabilitation experts to drive successful patient outcomes Provide education to patients and their families regarding therapeutic objectives, discharge plans, and necessary home exercises Document evaluations, treatments, and patient progress in accordance with facility policies and regulatory standards Participate actively in care team discussions and interdisciplinary meetings to ensure cohesive care delivery Education
Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution Certifications
State licensure as a Physical Therapist CPR certification preferred Additional specialized certifications, such as in geriatrics or neurology, are beneficial but not mandatory Skills
Prior experience highly preferred Ability to work with complex medical and mobility challenges Excellent communication and teamwork capabilities Proficient in digital record-keeping and clinical documentation systems Benefits
