Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor, this role is responsible for remote services including the interpretation, evaluation, preparation, and dispensing of medications pursuant to legitimate orders while following established policies, procedures, and standards of practice across all Brown University Health affiliates. The pharmacist collaborates with multidisciplinary providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost‑effective medication use, oversees pharmacy technicians and support personnel, and upholds the organization's core values of Compassion, Accountability, Respect, and Excellence. Responsibilities
Establishes and maintains compliance with Brown University Health's Flexible Work Arrangements Guidelines. Receives patient medication orders remotely when entered into the electronic health record (EHR) by a provider. Reviews and interprets each order for completeness and therapeutic appropriateness, considering the patient's clinical status, concomitant drug therapy, diet, and developmental stage. Contacts the provider or covering provider as necessary to initiate interventions to ensure therapeutic efficacy, minimize risks, and optimize cost‑effectiveness. Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective medication use for the patient populations served. Provides drug information and consultation to prescribers, nurses, and health‑care professionals on drug interactions, side effects, incompatibilities, allergies, medication discrepancies, stability issues, and administration. Proactively reviews and approves medication orders in the EHR, ensuring proper drug, dose, form, frequency, and duration. Ensures relevant patient parameters (e.g., diagnosis, allergies, weight, age) are present in the EHR. Identifies, documents, resolves, monitors, and prevents drug‑therapy related problems, including contraindications, duplications, and interactions. Engages with other clinical pharmacists or specialists for complex clinical situations. Integrates patient‑focused medication therapy management and pharmaceutical care following established pathways or guidelines. Participates in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications, recommending dose adjustments and lab monitoring as needed. Ensures compliance with target‑drug programs, formulary and medication‑management initiatives, and adverse‑drug‑event monitoring programs. Documents and reports all clinical interventions, medication errors, near misses, and adverse drug events per established protocols. Remotely reviews and approves sterile-product formulations requiring compounding or manipulation before dispensing. Completes in‑process and final checks for medications prepared by pharmacy technicians, ensuring accuracy, sterility, and stability of compounded products. Participates in continuous quality improvement activities and total‑quality‑management initiatives, including adverse‑drug‑reaction and medication‑incident reporting programs. Maintains professional competence through staff‑development and skills‑enhancement programs, independent study, and membership in professional organizations. Performs other duties as assigned. Minimum Qualifications
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Current unrestricted licensure as a licensed pharmacist or a licensed 90‑day temporary pharmacist in Rhode Island at the time of hire. Within 90 days of start date, maintain current unrestricted licensure as a licensed pharmacist in Rhode Island. Current licensure as a registered pharmacist in the Commonwealth of Massachusetts at the time of hire. Within 18 months of hire, obtain and maintain Board of Pharmacy Specialties certification in a clinical pharmacy specialty (e.g., Pharmacotherapy, Oncology, Cardiology). Successful completion of a Post Graduate Year 1 (PGY‑1) residency from an accredited program and/or a minimum of 3 years of clinical experience. Effective interpersonal and communication skills, both written and verbal, for interacting with colleagues and other health‑care professionals. Effective organizational and presentation skills. Knowledge of specialty‑specific technology, including pharmacy information systems, automated dispensing technology, software, equipment, and automation. Experience with Microsoft Word, Excel, Outlook, and a web browser. Work Environment and Physical Requirements
The pharmacist will establish a secure workspace environment to protect the confidentiality of sensitive information, including confidential information, protected health information (PHI), and protected identifiable information (PII). The employee is responsible for costs associated with setting up, furnishing, and maintaining their remote workspace, and for maintaining appropriate internet connectivity to access Brown University Health resources. The role involves extended periods of sitting and requires visual and manual dexterity to operate a computer, with occasional travel to Brown Health affiliates as needed. Pay Range
