Supervises work activity of assistant(s). Our community health based model allows our therapists to work in the Program of All-Inclusive Care for the Elderly (PACE) outpatient center and in participants homes as needed. Conducts initial assessments and periodic reassessments. Develops and routinely updates plans of care, regularly informing the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of each participant. Remains alert to pertinent input from other team members, participant caregivers. Documents changes in a participants medical record consistent with documentation policies/ph3Education/h3pMasters degree in physical therapy from an accredited university, except for applicants who graduated prior to 1/1/2003 and possess a Bachelors degree in physical therapy or a Bachelors degree and Certification in physical therapy./ph3Experience/h3pTwo (2) years working as a Physical Therapist. One (1) year experience working with the frail and elderly population preferred. If not met, willingness to train on the complexities of serving a frail and elderly population is required./ph3Other Requirement(s)/h3pActive State of Washington Physical Therapist license. Must have active CPR and First Aid Certification within 1 month of hire. Ability to travel within the Seattle area as needed. Valid Drivers License and Proof of Insurance required, if applicable./p/div
