Occupational Therapist

CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home care that promotes patient independence and safety. In this role, you will evaluate functional abilities, create and implement individualized treatment plans, and provide ADL training, home safety assessments, and adaptive equipment recommendations. You will collaborate closely with an interdisciplinary team to optimize patient outcomes, document care accurately, and educate patients and caregivers.
