*Responsibilities*
* Deliver comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions with precision and compassion.
* Administer medications and injections accurately, including IV infusions and specialized treatments such as botulinum toxin therapy, following all safety protocols.
* Assist with complex medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes using aseptic techniques.
* Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with diverse needs—including dementia care, Alzheimer's care, hospice support, and memory care.
* Manage documentation meticulously within Electronic Health Record (EHR) systems such as PointClickCare, ensuring compliance with HIPAA regulations and accurate medical records.
* Provide patient education on health coaching topics such as medication management, nutrition, discharge planning, and chronic disease management.
* Respond promptly to urgent situations involving acute pain management, airway management, ventilator support, or emergency triage in fast-paced environments like Level I trauma centers or emergency rooms.
*Requirements*
* Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS).
* Proven experience in diverse clinical settings such as hospital medicine, ICU, emergency medicine, long-term care facilities—including nursing homes and assisted living communities—and home & community care environments.
* Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.
* Demonstrated proficiency with EMR/EHR systems like Epic, Cerner or Athenahealth for accurate medical documentation and patient tracking.
* Experience working with individuals with disabilities or developmental disabilities; familiarity with dementia or Alzheimer's care is highly valued.
* Ability to perform technical skills such as phlebotomy, IV insertion and maintenance, sterile processing, specimen collection/processing, suturing, and use of medical imaging tools when necessary.
* Excellent patient assessment skills combined with the ability to handle high-pressure situations involving acute trauma or infectious disease care.
* Strong communication skills to effectively collaborate with healthcare teams across specialties including pediatrics (neonatal ventilator), geriatrics (geriatrics), behavioral health or occupational health settings. Pay: From $32.50 per hour
