Licensed Practical Nurse This position will provide individualized patient/resident care in relation to the following CORE concepts; Safety, Technology, Program and Services, Equipment, Documentation, Communication and Philosophy along with established Position Specific Competencies. LPN will display skills, knowledge and commitment necessary to comply with all established procedures, guidelines and SOPs found in CORE concepts and Position Specific Competencies. The duties are as follows; but not limited to:
Demonstrates knowledge and skills to carry out a full range of practical nursing care and specialized patient care duties to patients with a variety of physical and/or behavioral problems in the outpatient setting. Assists with patient education and patient groups regarding medications and disease management. Re-enforces all instructions previously given to patients by the MD/PA/NP/RN. Demonstrates ability to manage telephone care to include disease management, medication management, lab and radiology results and other telephone communication. Utilizes nursing protocols with attention to the individuality of patients, patient safety and evidence-based practice. Assists in care coordination and access to specialty care through communication and placing consult per nursing protocols.
Total Rewards of a Allied Health Professional Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66 Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)
