Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The setting may be a patient’s private residence or a room in an independent living facility. Duties include:
Supervision of Physical Therapy Assistants, temporary licensees, students, and support personnel.
Adherence to department and patient treatment schedules, timely clinical and billing documentation, and attendance at meetings.
Participation in patient care and staffing conferences.
Referral of patients to other disciplines and agencies.
Consultation with rehabilitation team members to select appropriate therapeutic regimens.
Adaptation of program and plan of care to patient needs.
Promotion of a safe environment and enforcement of fire and safety hazards elimination.
Ordering supplies and equipment as necessary.
Cleaning, maintaining, identifying, and reporting equipment repair needs.
Ensuring patient and family receive high quality, compassionate service.
Screening, examination, and evaluation of patients including history, systems review, and appropriate tests to establish diagnosis, determine precautions, and design treatment goals.
Providing therapies such as:
Therapeutic exercise (aerobic conditioning, balance, agility, body mechanics, postural stabilization, flexibility, gait, neuromotor development, relaxation, strength, power, endurance).
Functional training.
Manual therapy.
Airway clearance techniques.
Integumentary repair and protection.
Use of electrotherapeutic, physical agent, and mechanical modalities.
Health and wellness education.
Fall risk assessment and intervention.
Case management in home and community.
Design, fabricate, fit, and apply adaptive devices (orthotics, prosthetics) and train patients.
Select appropriate activities based on physical, mental, and cognitive capacity.
Teach patients needed skills for mobility activities and evaluate progress.
Perform routine reexamination to modify treatment plan.
Incorporate health and wellness programming into treatment plans.
Perform home and community mobility assessments and provide safe mobility recommendations.
Perform other related duties as required.
Qualifications
Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
Master’s degree in Physical Therapy; OR
Master’s degree in a related field plus Bachelor’s degree in Physical Therapy; OR
Bachelor’s degree in Physical Therapy plus five years of progressively responsible PT experience.
Degree earned outside the U.S. must be evaluated as equivalent to a U.S. PT degree.
Valid driver’s license and reliable transportation to patients’ residences.
Salary & Bonus Posted Salary Range: USD $63.00 - $63.00 /hr. Bonus: USD $5,000.00
