Assist Participants with activities of daily living including bathing, dressing, grooming, toileting, feeding, oral hygiene, and personal care. Support Participant mobility, transfers, repositioning, and ambulation using proper safety techniques and assistive devices. Monitor Participant condition and promptly report changes in physical, emotional, or functional status to nursing staff. Assist with therapeutic exercises and rehabilitation activities as directed by licensed staff. Encourage social engagement and participation in activities and daily routines. Prepare and serve meals according to Participant dietary needs and preferences. Monitor food and fluid intake as directed. Take and document vital signs including temperature, pulse, respiration, and blood pressure. Assist with delegated clinical support tasks within CNA scope of practice. Maintain accurate and timely documentation in the electronic health record. Follow infection control, PPE, and safety protocols in Participant homes and center settings. Maintain a clean, safe, and respectful care environment. Communicate effectively with nurses, therapists, caregivers, and interdisciplinary team members. Participate in care planning meetings, training, and quality improvement activities. Provide companionship, emotional support, and respectful Participant-centered care. Everyone's journey is unique. Friendly, patient, and comfortable working closely with seniors and caregivers. Reliable, compassionate, and attentive to Participant safety and wellbeing. Able to recognize and report changes in Participant condition. Comfortable assisting with mobility, transfers, and hands-on personal care. Team-oriented with strong communication and collaboration skills. Able to stay organized and follow through on assigned tasks. Committed to treating Participants with dignity, respect, and compassion. CPR and First Aid Certification (or willingness to obtain after hire - we'll provide the training!) Valid state driver's license with a clean driving record. Successful completion of background check, physical, drug screening, and TB screening. Must be at least 18 years of age. Education and Experience:
High School Diploma or equivalent required Current CNA certification in the state of practice required At least one year of experience in home care, long-term care, or a geriatric setting preferred Experience working with the frail or elderly population is a plus Physical Requirements:
Must be able to move intermittently throughout the workday. Must be able to lift, push, pull, and move Participants safely. Frequent standing, walking, bending, lifting, and reaching required throughout the workday. Must be able to assist Participants with mobility, transfers, and evacuations when necessary. May require travel between Participant homes, centers, or training locations. Must remain alert to Participant safety needs at all times.
