Conduct evaluations for patients in both inpatient and outpatient environments, crafting personalized treatment plans Implement physical therapy interventions that emphasize mobility, strength, balance, and overall functionality Work in collaboration with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes Guide patients and their families in understanding therapeutic goals, discharge strategies, and home exercise regimens Keep detailed records of evaluations, treatments, and patient progress in alignment with facility standards and regulations Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution Certifications
State licensure as a Physical Therapist CPR certification is advantageous Specialized certifications (e.g., neurological, geriatrics) are welcomed but not mandatory Skills
