POSITION IS LOCATED IN TRUTH OR CONSEQUENCES, NM The New Mexico State Veterans¿ Home is located on sixteen picturesque acres in Truth or Consequences, New Mexico. It provides long-term and skilled nursing services for honorably discharged veterans, their spouses, or parents of veterans killed in combat action. The facility has 131 nursing home, skilled-nursing living beds. Services for residents are provided 24/7, focusing on maintaining or improving their physical, mental, and psychosocial well-being under a personalized plan of care developed by licensed health professionals
Why does the job exist? The Transporter Nursing Aide is responsible for providing safe transport, supervision, and hands-on personal care for residents during travel to and from medical appointments, in-house clinical services, therapy sessions, and other facility activities. The Transporter Nursing Aide works as part of a team under the direction of the Transportation Scheduler and Charge Nurse, assisting with activities of daily living, monitoring resident well-being, and maintaining a safe environment during transport and throughout the facility.
How does it get done? Shift Assignments & Reporting Receive assignments from the Transportation Scheduler. Report to the Charge Nurse or Lead Aide any deviations from normal resident routines or condition. Communicate promptly any resident concerns, complaints, or follow-up needs to the Transportation Scheduler. Document all care provided during transport and other assigned duties. Resident Transport & Safety Assist residents in preparing for transports, including oral care, dressing, and ensuring they have all necessary medical equipment and personal items. Assist residents in boarding, exiting, and securing wheelchairs, Geri-chairs, walkers, and other mobility devices. Safely transport residents to and from appointments, therapy sessions, and other facility activities. Assist residents with personal care as needed, ensuring cleanliness, proper clothing, and necessary medical appliances. Promote resident independence through cueing, set-up, and support with ADLs. Meal Support & Dining Assistance Offer fluids, assist with meals, and provide reassurance and companionship during transport and care. Assist residents with eating as needed and record dietary intake and meal percentages. Ensure cleanliness of resident, chairs and equipment after meals. Resident Monitoring & Rounds Reposition residents at least every two hours and assist with passive range-of-motion exercises as assigned. Check incontinent residents at least every two hours, providing toileting and perineal care as needed. Environmental & Equipment Maintenance Maintain a safe environment by checking vehicles, wheelchairs, lifts, and other transport equipment for proper function and cleanliness. Report maintenance needs and prepare work orders for repairs. Clean and maintain unit equipment, linens, utility rooms, refrigerators, food cabinets, and nurse stations. Training & Professional Development Precept and mentor new nursing assistants as assigned. Attend required in-services, continuing education, defensive driving, and annual training. Participate in a minimum of 12 hours of continuing education per year, as required by facility policy. Additional Responsibilities Provide direct resident care on units when not assigned to transport duties which may include, but not limited to personal care, assisting with eating, vital signs, ambulation, repositioning, and assisting with activities of daily living under the direction of the Charge Nurse. Perform other duties as assigned to support resident care and facility operations. Maintain flexibility to assist wherever residents need care, ensuring continuous resident support and smooth facility operations.
Who are the customers? The primary customer are the brave men and women that answered our nations call, executed their duties and now are unable to live in their respective communities due to failing health. This job will also support the widows of the service men and women to include parents that have lost children in combat (Gold Star Parents)
Ideal Candidate Follows the chain of command and facility policies at all times. Expected to work independently after probation, managing workload, performance, and time efficiently. Conducts oneself professionally and maintains adherence to the Nursing Department dress code. Demonstrates teamwork, cultural awareness, and effective communication with residents, families, and co-workers. Frequent lifting, pushing, and assisting residents. Ability to stand, walk, and assist residents for extended periods. Ability to respond quickly and safely in emergencies. Work hours may vary based on resident transport and service schedules.
Minimum Qualification Certification as a Certified Medication Aide (CMA), Home Health Aide (HHA), or Certified Nursing Aide (CNA) and one (1) year of experience providing basic dietary, housekeeping, and/or laundry services in a traditional long term care facility and/or as it pertains to the essential duties and responsibilities of the classification. Any combination of education from an accredited college or university in a related field above certification requirements and/or direct experience in this occupation totaling one (1) year may substitute for the required experience.
Employment Requirements This position is designated safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules, and regulations in relation to alcohol and/or drug testing. Must possess and maintain a valid American Heart Association BLS / CPR Certification Employment is subject to criminal background check pursuant to New Mexico Caregivers Criminal Screening Act, (Sections 29-17-2 through 29-17-5 NMSA 1978 and 7.1.9.1 through 7.1.9.11 NMAC) and is conditional pending results. Must possess and maintain a valid New Mexico driver's license.
Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment. Successful completion of Resident Transport training is required upon hire. Must possess and maintain a valid Safe Food Handling Certificate or receive within 1 week of date of hire. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA) training. Must sign and observe confidentiality agreement. Must be able to read, write, speak, and comprehend the English language. Occasional travel for training & meetings and driving a State of NM owned motor vehicle may be required; Working hours may include weekends, holidays, overtime, and callback hours. Working Conditions Work is performed in an office setting within a long-term facility with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Walking throughout the facility and frequent communication and interaction with residents, staff, and the public. Periods of walking, standing, bending pushing, pulling, lifting, and stretching for a variety of activities may be required. Must be able to function under pressure and stressful situations Health hazards such as exposure to contagious viruses and diseases. May also include routine exposure to public, fellow employees, and facility resident illness, stressful situation, and bodily fluids, as well as exposure to physical and/or verbal abuse. Occasional travel for training & meetings and driving a State of NM owned motor vehicle may be required; some lifting; extended hours to meet deadlines will be required. Working hours may include on-call, nights, weekends, holidays, overtime, and callback hours.
