Join a dedicated healthcare team in the heart of the Midwest region, providing compassionate and direct patient care in an acute rehabilitation and transitional care facility setting. Under the guidance of licensed nurses, you will play a vital role in supporting patients' daily needs and ensuring their safety and comfort. General Summary Working under the supervision of an RN, LPN, or shift manager, the Patient Care Assistant (PCA) delivers essential bedside care and assists with activities of daily living to promote patient well-being and recovery. Essential Duties and Responsibilities
Adhere to established safety protocols including infection control, safe patient handling, and timely communication of patient condition changes to licensed nursing staff. Perform basic care tasks such as monitoring vital signs, conducting glucometer checks, assisting with toileting, feeding, bathing, and mobility activities. Collaborate closely with nursing personnel to meet patient needs promptly and enhance the overall patient experience. Accurately document all care activities to support clinical decision-making and continuity of care. Effectively communicate relevant patient information to the healthcare team to improve outcomes and quality of care.
Education High school diploma or GED is preferred. Experience At least one year of experience as a Certified Nursing Assistant (CNA) is preferred. Certification and Training If uncertified or with less than one year of experience, candidates must complete a PCA training academy. American Heart Association Basic Life Support (BLS) certification is required within the first 90 days of employment; training is provided. Mental and Physical Requirements This role requires significant mental focus and physical activity including lifting up to 35 pounds, standing, walking, bending, and other movements necessary for patient care. Working Conditions Frequent exposure to infectious and communicable diseases through blood and bodily fluids is expected. Occasional encounters with hostile individuals may occur.
