Responsible for direct patient care and case management of Hospice patients in outpatient settings. Ensures that end-of-life care is delivered within quality and utilization guidelines and is directed toward the achievement of desired clinical outcomes for patients and their families.How you grow, learn and thrive matters here.

Qualifications:
Associate Degree in Nursing
Registered Nurse Licensure - Compact/Multi
Minimum one year in direct patient care setting or home health care.
Valid Driver's License
BLS required at time of hire
