Compensation:

• Start of Care (SOC): $260 per visit
• Evaluation (No SOC): $120 per visit
• Supervisory Visit: $85 per visit
• Re-Certification Visit: $100 per visit
• Follow-Up/Routine Care (RC): $85 per visit
• Death Visit: $150 per visit
• PRN Visit: $180 per visit
• Interdisciplinary Group (IDG) Meeting: $45 per meeting
• In-Service Training: $45 per session
• Discharge Visit: $85 per visit
• Declined/Missed Visit: $25 per event (status must be provided at the time of the event)

Responsibilities:

• Comprehensive Patient Assessment: Conduct thorough evaluations of patients' physical, emotional, and psychosocial needs to develop individualized care plans.
• Symptom Management: Monitor and manage patients' symptoms, administering medications and treatments as prescribed to ensure comfort and quality of life.
• Patient and Family Education: Provide education to patients and families about disease progression, symptom management, and end-of-life care options.
• Emotional Support: Offer compassionate support to patients and families, addressing concerns and facilitating discussions about goals of care.
• Coordination of Care: Collaborate with physicians, social workers, chaplains, and other members of the interdisciplinary team to ensure cohesive and comprehensive care delivery.
• Documentation: Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in compliance with regulatory standards.
• Emergency Response: Respond promptly to medical emergencies, providing appropriate interventions and support.
• Quality Improvement: Participate in quality improvement initiatives to enhance patient care and organizational performance.

Qualifications:

• Current Registered Nurse (RN) license in the state of California.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Strong clinical assessment and critical thinking skills.
• Excellent interpersonal and communication abilities.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation.
• Proficiency in electronic medical record (EMR) systems is preferred.
