Shift: 7:00 AM - 5:00 PM
Experience:
• 10 months of role experience is required with some in the last 12 months.
• Travel experience is required from any number of months.
• 3 months of experience with Epic is required from any number of months.
• Trauma Level 2 experience is preferred from any number of months
Requirements:
• This role may require floating to additional units and locations
• Travel only, local not allowed. Candidates must live >50 miles from facility to be submitted.
• Flu vaccination required after submission. Religious and medical declinations accepted.
• 6 months gap required between for Staff at Program: Aya Program - Providence Health & Services and no current placement allowed at Program: Aya Program - Providence Health & Services.

Additional Details:
References must cover at least 1 full year of relevant experience in specialty within the past 3 years
Cannot have any hits/suspensions on license or any background hits from any point in the past.
Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.
ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)
Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
Required Skills/Experience:
- A CVT with experience in stroke/cerebral angiograms, cardiac, and IR is preferred. We need someone who is an experienced traveler (no first-time assignments, please).
- Diagnostic Cardiac Cath lab, Interventional Cardiac Cath lab, Cardiac Cath Lab Procedures, Cardiac Implant Closure Device, Cardiac Stent Placement, Diagnostic Catheterizations Adult, IABP Placement/Removal, Interpretation of dysrhythmias, Left heart catheterization, Percutaneous Coronary Interventions (PCI), Percutaneous Transluminal Coronary Angioplasty (PTCA), Right heart catheterization, Rotablader athrectomy, Scrub Procedures, Procedural Responsibilities, Equipment set up, Recognition of life-threatening arrhythmias, Room circulator, Scrub- maintain sterile field, Infection Control, Isolation Precautions/PPE, Sterile Technique, Age specific competency, Middle Adult, Older Adult.
Unit.Facility Details: All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
Special Requests:
- RTO must be listed at time of submission.
- RTO - 6 days total or 3 consecutive days can be auto-approved. Anything more, traveler will sign contract with pending RTO. *Anything more than 6 Days will potentially delay your offer while we run approval by the client. To minimize turn around please limit total RTO requests. Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD)
- MUST request all RTO / specific scheduling requests up-front during submittal. Post submittal/offer RTO may not be considered or granted. Inflexibility beyond contracted schedule and/or RTO may lead to termination of contract and system DNR.
- You must be able to complete labs within 48 hours of signing.
- Pre-Approved Time Off: 2 Requests
- Must have a vehicle for this assignment.
Shift & Scheduling: Call Required: Yes, 1 night per week, approx. 1 wknd (Fri-Mon) every 5
Floating Requirements: Travelers may be expected to float between sister facilities nearby your contracted facility. Travelers’ refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.
5 days maximum time off
2 references from last 3 years (Manager/Supervisor reference type) - required for submission
