About
We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will be responsible for assessing, planning, and implementing individualized treatment programs for patients with various physical impairments. This role requires a strong understanding of anatomy and physiology, as well as the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
*Responsibilities*
* Conduct thorough evaluations of patients' physical abilities and limitations.
* Develop and implement personalized care plans based on individual patient needs.
* Provide therapeutic interventions, including physiotherapy techniques, trigger point therapy, and rehabilitation exercises.
* Educate patients and their families about treatment plans and home exercise programs.
* Document patient progress using EMR systems and maintain accurate medical records.
* Collaborate with interdisciplinary teams in acute care, inpatient, outpatient, and home health settings.
* Participate in discharge planning to ensure continuity of care post-treatment.
* Stay updated on best practices in physical therapy and participate in ongoing professional development.
*Skills*
* Strong knowledge of medical terminology, anatomy, and physiology.
* Experience in geriatrics, , and acute care settings is preferred.
* Proficient in developing care plans tailored to individual patient needs.
* Familiarity with EMR systems for efficient documentation and record keeping.
* Excellent patient care skills with a compassionate approach to treatment.
* Ability to work effectively in both clinic and home & community care environments.
