About
The Care Transitions Liaison acts as a bridge between Aveanna, physicians, clients, hospitals, medical staff, and other healthcare professionals. This role serves as an advisor to clients, utilizing available resources and services to meet their needs and resolve issues. The Liaison evaluates services provided and community satisfaction based on client feedback and medical personnel input. Essential Job Functions
Collaborates with physicians, medical staff, and clients in assigned territories to assess satisfaction with home care services. Develops plans to address any dissatisfaction with services, presented verbally or in writing. Communicates with discharge personnel in assigned hospitals and facilities to ensure smooth patient transfers to Aveanna services. Educates physicians, discharge planners, and social workers about Aveanna services and the benefits of home health and hospice care. Identifies new physicians and discharge planners who may refer patients to Aveanna, establishing relationships through cold calls and joint meetings. Conducts initial and ongoing needs assessments of customers in assigned territories, responding accordingly or referring to relevant departments. Attends liaison meetings, participates in agency committees, and assists in enhancing meeting effectiveness. Creates educational materials and presents them to customers, promoting Aveanna services. Collaborates with Health Care Liaison team members on co-marketing activities and coordination efforts. Maintains accurate records of referral contacts and files for assigned locations. Remains informed about market trends and competing agencies in assigned territories. Meets or exceeds referral and admission quotas for assigned locations, using creative and cost-effective strategies. Requirements
Minimum two years of experience in healthcare, home health, or hospice sales is required. A valid active LPN license in the state of application is necessary. Familiarity with standards of practice for all services provided is essential. An active and unencumbered professional clinical license in the assigned territory is required. A valid driver's license with evidence of automobile liability insurance is mandatory. Aveanna complies with all health-related requirements in relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons.
