We are currently seeking a skilled and caring *Physical Therapist* to join our growing home health team.
*Position Summary*
The Home Health Physical Therapist is responsible for evaluating, planning, and implementing individualized therapy services for patients in their homes. This role offers autonomy, flexibility, and the opportunity to make a real impact in patients’ lives.
*Responsibilities*
* Perform initial and ongoing patient assessments to determine functional status, safety, and therapy needs.
* Develop and implement personalized care plans in collaboration with physicians, nurses, and other care team members.
* Provide therapeutic exercises, gait training, balance activities, and education to patients and caregivers.
* Monitor patient progress and adjust care plans as necessary to optimize outcomes.
* Educate patients and their families on treatment options, home exercises, and self-management strategies.
* Collaborate with multidisciplinary teams to ensure cohesive patient care across various settings such as acute care, outpatient clinics, and home health environments.
* Maintain accurate documentation of patient evaluations, treatment sessions, and progress notes using EMR systems.
* Utilize techniques such as trigger point therapy and orthopedic rehabilitation to enhance recovery processes.
* Participate in discharge planning to facilitate smooth transitions for patients from hospital to home or community settings.
*Experience*
* Proven experience in physiotherapy within inpatient, outpatient, or home health settings is preferred.
* Strong knowledge of physiology, anatomy, medical terminology, and rehabilitation practices.
* Familiarity with discharge planning processes and the ability to create effective care plans for diverse patient populations.
* Experience working with geriatrics is highly desirable.
* Previous exposure to acute care environments is a plus.

Job Types: Full-time, Part-time
Pay: $50.00 - $70.00 per hour
Expected hours: 20 – 40 per week
