Primary Care Physician

Oak Street Health seeks a Primary Care Physician to provide comprehensive, patient-centered care to adults on Medicare in underserved communities. In this role, you’ll deliver preventative, evidence-based primary care, manage chronic conditions, and coordinate with an interdisciplinary team to enhance patient outcomes. You’ll build longitudinal relationships, support care planning, and engage in quality improvement initiatives. Spanish language skills are strongly preferred to better serve our diverse patient population.
