20–25 per week
Position Overview Support participants directly through intake, assessment, referrals, and follow‑up. Ensure participants are connected to services and support, and maintain accurate program records.
Responsibilities
Conduct participant intake and assessments
Develop individualized support plans
Coordinate referrals and follow‑ups
Maintain participant records and reporting
Support Program Managers in service delivery
Qualifications
Experience in case management, social work, or nonprofit service delivery
Strong interpersonal and communication skills
Organized and detail‑oriented
Comfortable managing multiple participants and tasks simultaneously
