divh2Hospice Certified Nursing Assistant/h2pSalary: $19-22/hr BOE/ppSchedule: Full-time | Monday-Friday/ppShift Details: 8 AM-4:30 PM/ppLocation: Oregon, Ohio, and surrounding areas/ppRequirements of the CNA role:/pulliLicensed Nursing Assistant in the state of practice (CNA)/liliCPR/BLS Certified/liliPrior experience working in an AL/LTC/SNF facility or Home Health REQUIRED/liliEnjoy working with Senior Citizens, Positive Attitude, Professional, Responsible Dependable/liliMust be willing to travel/liliValid drivers license and Auto insurance Coverage with reliable transportation/liliHospice experience preferred/li/ulpAbout the Certified Nursing Assistant role:/pulliProvide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care./liliTravel to patients residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life./liliProvide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments./liliAssist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care./liliSafely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts./liliObserve and report changes in the patients physical condition, emotional status, or home environment to the case manager or supervising nurse./liliAccurately document care provided in accordance with Care 360 Hospice policies and procedures./liliFoster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences./liliProvide support and education to family members and caregivers within the scope of the CNA role./liliParticipate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance./liliAll other duties as assigned/li/ulpWhy work with Care 360 Hospice:/pulliMedical, dental, and vision health Insurance/liliPaid Time Off/lili401(k) Matching/liliLife Insurance/liliGrowth opportunities/li/ulpCare 360 Hospice/pp3130 Executive Parkway, Suite 500/ppToledo, OH 43606/ppCare 360 Hospice is an Equal Opportunity Employer M/F/D./p/div
