A. / Transportation Professional/h2pDepartment - Assisted Living (ALC) Skilled Nursing (DSH)/ppReports to - Assisted Living Administrator and DSH Administrator/ppPosition Purpose - The ALC DSH CNA/Transportation Professional supports Villagers during medical appointments by advocating for their needs, assisting with the appointment process, and empowering them to make informed decisions about their care. This role ensures Villagers receive the necessary support while respecting their autonomy in healthcare choices./ppRequired Qualifications/pulliCertified Nursing Assistant (CNA) in good standing, meeting all relevant Georgia and federal requirements/liliHigh school diploma or GED/liliProficiency in English (read, write, follow instructions)/liliValid Class C Georgia Drivers License, proof of insurance, and acceptable driving record/liliSuccessful completion of background check, drug screen, and TB screening/liliCPR/First Aid certification/liliPhysical capacity to lift up to 25 lbs independently and assist up to 75 lbs with help/liliAbility to assist Villagers into and out of vehicles safely/li/ulpKey Responsibilities/ppstrongTransportation Medical Appointment Support (Primary Focus)/strong/pulliProvide safe, timely, and compassionate non-emergency medical transportation (NEMT) for Villagers to off-campus doctor visits, therapies, labs, hospital outpatient appointments, pharmacy runs, etc./liliUse accessible practices (ramps, lifts, securement) as needed, ensuring that each Villagers mobility needs are addressed and accommodated/liliOffer emotional support, reassurance, and assistance to Villagers before, during, and after appointments to reduce stress and promote positive experiences/liliAdvocate on behalf of Villagershelp them communicate needs or concerns to medical staff, assist with check-ins, accompany them as needed/liliMaintain detailed trip logs, mileage records, and required documentation of each outing/liliMonitor vehicle condition: ensure cleanliness, appropriate fuel levels, safety checks, and report any mechanical or safety concerns/liliPlan routes to optimize time and safety (considering traffic, Villager comfort breaks, accessibility)/liliCoordinate pick-up and drop-off times with the care team to minimize waiting and overlap/liliFollow all vehicle, traffic, and safety regulations; secure mobility devices, seat belts, restraints appropriately/li/ulpstrongSupport Role in Care (Secondary, When Off-Transport Duty)/strong/pulliWhen not transporting, assist as needed with personal care tasks such as grooming, toileting, dressing, positioningalways in a supportive role and in a way that honors Villager choice/liliBe available to help fill in gaps in care, offer respite support, or respond to situations during non-transport periods/li/ulpstrongCommunication Documentation/strong/pulliObserve Villagers physical, emotional, or behavioral changes during transport or in supportive moments; report concerns promptly to nursing or administration/liliDocument transportation interactions, appointments attended, outcomes, challenges, and any follow-up needs clearly and promptly/liliCommunicate effectively with the care team (ALC and DSH) so everyone knows appointment outcomes, delays, or Villager feedback/li/ulpstrongSafety, Infection Control Environment/strong/pulliAdhere strictly to Universal Precautions, infection control protocols, and safety procedures, especially when assisting Villagers in vehicles/liliUse proper body mechanics when assisting with transfers into/out of vehicles/liliMaintain hygienic and orderly Villager spaces and vehicles; report repair or cleaning needs/liliEnsure vehicles are safe and properly clean; monitor maintenance, schedule repairs as needed/li/ulpstrongVillager Rights, Dignity Relationship Building/strong/pulliTreat every Villager with kindness, respect, and patience; remain attuned to their emotions and preferences/liliProtect Villager privacy and choices; never rush or force/liliPromote Villager autonomylisten to preferences about timing, seating, reinforcement, and comfort/liliBuild trust and a reassuring presence, especially during transport when Villagers may feel anxious/li/ulpstrongOther Duties/strong/pulliAssist with admissions, transfers, and discharges as directed (transport or supportive role)/liliPerform additional tasks when needed, always keeping Villager safety, dignity, and comfort in mind/li/ulpPhysical Operational Requirements/pulliFrequent use of hands, arms, walking, bending, reaching/liliAbility to sit, stand, walk, climb, kneel, balance, and reach in tight vehicle spaces/liliMust be able to lift 25 lbs independently and assist with up to 75 lbs/liliMust have good vision, hearing, depth perception, and ability to drive/liliMust be able to perform CPR / First Aid if needed/liliMust safely assist Villagers into/out of vehicles, using mobility aids, lifts, or ramps/liliAbility to safely operate and monitor transportation vehicle systems/li/ul/div
