VNS Health is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team, providing high-quality, patient-centered care in home and community settings throughout New York City. As a key member of an interdisciplinary care team, you will work under the supervision and direction of a licensed Physical Therapist to carry out individualized treatment plans that help patients improve mobility, strength, balance, and overall function so they can safely remain in their homes and communities. You will conduct assigned therapeutic exercises and functional mobility training, assist with gait and transfer training, reinforce patient and caregiver education on home exercise programs and safe body mechanics, and carefully observe and report patient responses and changes in condition to the supervising Physical Therapist and clinical team. You will document all care in accordance with agency, state, and regulatory standards, using electronic health records to ensure accurate, timely, and compliant charting. In this role, you will collaborate closely with nurses, social workers, occupational therapists, and other clinicians to support comprehensive care plans rooted in compassion, integrity, and respect for each patient’s goals and cultural background. You will communicate clearly with patients and families, providing encouragement and education that empowers them to participate actively in their rehabilitation.
